INCOME SELF-CERTIFICATION FOR THE ELECTION OF

BOARD MEMBERS REPRESENTING THE LOW INCOME POPULATION

Delaware Opportunities Inc. is holding an election for representatives to the Board of Directors.  The purpose of having local people elected to serve on the Board is to have input from area residents.

Board members are responsible for policy making decisions for the agency and for maintaining and evaluating program effectiveness.  Anyone may be elected to serve as a representative of the low income on the Board; however, only those residents whose gross income is at or below the following poverty level will be permitted to vote.

The income levels used to determine eligibility are as follows.  The incomes stated are gross incomes, before all tax deductions have been made.

After reading the above statement, I certify that my income is within the above guidelines and I am a resident of Delaware County and I am, therefore, eligible to vote in this election.
