To the fullest extent permitted by law, I

agree to hold harmless Kankakee Community College, its elected and appointed officials, employees and volunteers and others working on behalf of the scheduled activity/event; against any and all claims, demands, suits, loss, including all cost connected therewith, for any damages which may be asserted, claimed or recovered from Kankakee Community College, its elected and appointed officials, employees, volunteers and others working on behalf of Kankakee Community College, by reason of personal injury, including bodily injury and death; and/or property damage, including loss of use thereof, which arises out of or is in any way connected or associated with this contract.

Typically, Kankakee Community College (KCC) students should be transported to events in KCC owned vehicles. If I am a currently enrolled KCC student or KCC staff or faculty member and choose to transport other persons or students in my personal vehicle to this activity, I take full responsibility for their safety. I certify that I have adequate insurance on my personal vehicle, my personal vehicle is in good operating condition and I agree that I shall indemnify and Hold Harmless Kankakee Community College and its release against any claim brought against them by my passengers. I shall also provide to Kankakee Community College a copy of proof of insurance on my motor vehicle that specifically includes coverage limits. Further, I will not possess or use alcohol or unlawful substances or substances that may cause drowsiness while participating in this activity. I also agree to:

1) Observe all federal, state and local laws as well as Kankakee Community College rules and regulations at all times.

2) Conduct oneself in a mature and responsible manner in accordance with the KCC Student Code of Conduct.

3) Attend and participate in all activities, meetings or sessions as allowable given any physical, medical or mentally covered protections and accommodations

By signing this form and participating in this activity, you will be releasing all claims for injuries that you might sustain in connection with this activity. You also grant the college representative the right to authorize medical care, if the emergency contact cannot be reached.
