I hereby authorize my Athlete to participate in the Coastal Virginia Cheer Athletics (CVCA) program. I confirm that I fully understand the obligations required of both myself and my Athlete, including the necessity for personal health insurance coverage. I also consent to my athlete receiving medical treatment in instances where I cannot be present or contacted. I understand that a non-refundable registration fee of $150, must be paid upon submission of this form to secure my athlete's participation in the program.

Additionally, I recognize the critical importance of my athlete's consistent attendance at every practice, as all competitions are mandatory. In cheerleading, there is no "bench" to pull from; every team member's participation is essential.

Medical Authorization:

I authorize Coastal Virginia Cheer Athletics (CVCA) and its representatives to consent to medical treatment for my athlete when I cannot be reached. This authorization does not require a prior determination of a life-threatening emergency or the risk of serious or permanent injury resulting from treatment. 

Risk Acknowledgment:

I understand that cheerleading and tumbling involve risks, including the potential for serious injury. I am fully aware that activities involving motion, height, or athletic exertion carry inherent dangers. I agree to hold CVCA, along with its directors, coaches and staff, harmless from any injuries or resulting expenses. I release and discharge all rights and claims against CVCA related to such injuries.

Media Release:

Permission is granted to CVCA to use and publish photographs of each athlete for promotional purposes on the program's website, social media platforms, and in promotional materials. No compensation will be provided for use of these images. It is acknowledged that CVCA will not publish the athlete's name, age, or other identifying details alongside these photos, unless granted specific permission from the parents.

In consideration of {athleteName}, being allowed to participate in any way in CVCA program, related events, competitions and activities, the undersigned acknowledges, appreciates and agrees that:

Acknowledgment of Risks:

Cheerleading is a physically demanding sport that includes height and rotation within a unique environment, posing significant risks of injury. These include, but are not limited to, death, severe neck and spinal injuries, and significant injuries to bones, joints, and muscles. While safety equipment and coaching can reduce these risks, they cannot be completely eliminated.

Assumption of Risk and Responsibility:

I, and {athleteName}, knowingly and freely assume all such risks, both known and unknown, even those arising from the negligence of Coastal Virginia Cheer Athletics (CVCA) or others. I accept full responsibility for My Athlete’s participation in CVCA programs.

Compliance and Monitoring:

I agree to comply with all standard and customary terms and conditions for participation in CVCA programs. Should I observe any significant concerns regarding my Athlete’s readiness for participation or about the CVCA program itself, I will immediately withdraw my Athlete from participation and notify the nearest CVCA personnel.

Release and Waiver:

I, along with, {athleteName}, and on behalf of our heirs, assigns, personal representatives, and next of kin, hereby release CVCA, its employees, coaches, agents, subcontractors, other program participants, sponsoring agencies, sponsors, advertisers, and, if applicable, owners and lessors of premises used for CVCA programs (collectively referred to as "Releases"), from any and all injuries, disabilities, deaths, losses, or damages to personal property related to My Athlete’s involvement in CVCA programs, whether arising from the negligence of the Releases or otherwise, to the fullest extent permitted by law.

Indemnity:

I further agree to indemnify, defend, and hold harmless all of the above Releases from any and all liabilities arising from My Athlete’s participation in CVCA programs, even if due to the negligence of Releases, to the fullest extent permitted by law.

Consent:

I have read this Waiver and Release of Liability, fully understand its terms, and acknowledge that I am signing it voluntarily.

Medical Release:

In the event of an accident where medical treatment appears necessary, we will first attempt to contact the emergency contact(s) provided. If the emergency contact(s) cannot be reached, I hereby authorize Coastal Virginia Cheer Athletics (CVCA) to secure necessary medical treatment for {name} in any emergency that may arise while on the CVCA premises or at any associated event.

By signing, you confirm that you have read and agree to the information provided and will commit to adhering to CVCA's policies as outlined above.

Payment Deadline and Late Fees:

Registration payment of $150 is due at registration and secures a spot on the team. This fee is non-refundable.

Subsequent payments are due on July 1, August 1, and September 1. If payment in full is not made by September 1, the athlete will not be permitted to participate in the program and incur a $15 per week late penalty. 

CVCA accepts electronic payments and personal checks. Checks should be made payable to Coastal Virginia Cheer Athletics.

CVCA will charge a $30.00 return check charge for checks returned for any reason.

Two returned checks will result in payment accepted online only.

Payments encompass all costs associated with participation in the Coastal Virginia Cheer Athletics (CVCA) program, including registration, competition fees, choreography and music fees, uniform packages, and any other relevant fees. Please note that all payments, including installments, fees, and voluntary donations, are non-refundable. Travel costs are also not covered.

Commitment Acknowledgment:

I acknowledge the commitment, standards, and financial obligations involved in joining a team and becoming a member of CVCA.

Seasonal Commitment and Refund Policy:

I commit to participating in CVCA for the entire cheerleading season. I understand that if I voluntarily withdraw my Athlete, or if they are removed from the program for any reason, all incurred fees, including but not limited to choreography, coaching, competition fees, uniforms, practice wear, and tuition, are non-refundable.

Delinquency Consequences:

Should my account become delinquent, my Athlete may be required to sit out of practices and competitions until the account is settled. Accounts more than two months delinquent may result in dismissal from the program and potential legal action, with all associated legal fees charged to the responsible party. Returning the following season will require full upfront payment without the option for a payment plan.

Fundraising Credits:

Any credits or overpayments from fundraising will be forfeited if my Athlete withdraws or is removed from the program for any reason, due to the specific policies governing many fundraising activities.

Adherence to Policies:

{athleteName} and I will adhere to all policies and standards set forth by CVCA. Failure to comply may lead to restricted access to CVCA facilities or withdrawal of my Athlete from the program.

By signing this registration, you acknowledge that you have read and understood the CVCA Handbook (link below). Please complete the following steps to finalize your registration:

1. Email a copy of your Athlete's birth certificate to covacheer@gmail.com.
2. Return to CVCA website to pay registration.
3. Save this link for Parent Documentation, containing CVCA Handbook and season schedule information.

Once CVCA has received this signed registration form, registration payment, and birth certificate, your Athlete will be officially registered.
