PLEASE NOTE: All Applicants aged 16 years and older are required to sign the Scholarship Application Form. A parent/guardian's signature is also required for all performers UNDER the age of 18 Years Old.

And that you understand that becoming an applicant creates a binding agreement to these terms and conditions.
