KinHub Registration 

This form is for families enrolling to participate in KinHub for the term. It helps us collect the information we need to support your child’s participation, communicate with your family, manage invoicing and meet our safety, insurance and child safe obligations.The information you provide will be kept confidential and used only for program administration, participant support and service delivery. If you are an NDIS participant (self/plan managed) you will also be sent a separate service agreement.

    WHAT'S INCLUDED

    • 10 x 3hr facilitated group sessions
    • Professionally facilitated programs led by experts in areas such as drama, visual arts, music, film making, photography, basketball, OT and more (2-3/session)
    • All materials
    • First access to KinHub offsite activities + opportunities
    • Access to the KinHub community/networks
    • Connection and informal support for parents/carers
    • Supported group environment to help young people build positive peer connections
    • Access to outdoor park space for movement, sensory breaks + informal play

    SHARED RESPONSIBILITIES

    KinHub works best when families help create the kind of community we all want to be part of. Participation in the program includes:

    • upholding KinHub’s values and contributing to a respectful, inclusive community
    • actively supervising and supporting your child where needed, helping maintain the safety and wellbeing of the group
    • following KinHub’s Constitution, Code of Care and relevant policies, available at kinhub.org.au
    • ensuring any attending adult attending with your child holds a valid Working With Children Check 
    • letting KinHub know about any important changes to your child’s participation, support needs or contact details
    • paying program fees within the required timeframe

    KinHub plans staffing, materials and group continuity in advance. Enrolment is ongoing from term to term unless two weeks written notice is given before the next term.

    By enrolling, I agree to uphold KinHub’s policies and procedures and understand that KinHub is a family participatory, term based program. Parents/carers/support workers remain onsite and are responsible for individual supervision where needed. Enrolment secures a place for the full term, non attendance does not reduce fees and invoices are payable within the required timeframe. All attending adults require a valid Working With Children Check (WWCC).
