SECTION 6 - Teen Reach Adventure Camp - CONSENT FORM
COUNSELORS/STAFF: Every effort will be made to contact the camper's camp counselor from Royal Family KIDS Camp so consistency may be achieved from camp-to-camp. However, this is not always possible. All camp counselors and staff at Teen Reach Adventure Camp are interviewed, screened with background checks/fingerprints, and receive training before volunteering on the T.R.A.C. Team.
CONFIDENTIALITY: Every youth registered at T.R.A.C. has the right to confidentiality. Our camp counselors are instructed not to take photos or exchange addresses, phone numbers, email addresses, or social media accounts with any camper. Any photographs or videos obtained by T.R.A.C. will be edited, by blocking out faces, before being used in any promotional materials.
SAFETY: Every precaution will be taken to protect campers and volunteers from harm.
LIABILITY: T.R.A.C. volunteers are aware of our responsibility to keep each youth safe but Teen Reach/Teen Reach Adventure Camp/T.R.A.C. is not liable for injuries/death youth or volunteer staff may incur while at camp or participating in T.R.A.C. activities.
MEDICATION: Each day of T.R.A.C., medication will be administered to youth as directed by their physician. Teen Reach Adventure Camp will do everything in its power to prevent incorrect administration of medications. However, T.R.A.C./Teen Reach is not liable for incorrect medication provided by the legal guardian, incorrect dosages given, nor is it liable for wrong labeling on medicine bottles. Legal guardians are responsible for providing the correct medication, bottles and dosages at the time of check-in. THIS IS NOT THE TIME TO GIVE MEDICATION VACATIONS to your youth.
FINANCIAL COMMITMENT: T.R.A.C. is made possible by the volunteer hours and donations committed to running this program. T.R.A.C. is free for the caregivers and campers attending.
PARTICIPATION AGREEMENT: At T.R.A.C., campers and volunteers will participate in activities such as high ropes course, boating, kayaking, swimming, and archery, among other activities.
I acknowledge that participation in Teen Reach Adventure Camp and the activities described above involves risk to the participant (and to the participant’s parents or guardians, if the participant is a minor), and may result in various types of injury including, but not limited to, the following: sickness, bodily injury, death, emotional injury, personal injury, property damage, and financial damage.
In consideration for the opportunity to participate in the activity described above (the “activity”), the participant (or parent/guardian if the participant is a minor) acknowledges and accepts the risks of injury associated with participation in and transportation to and from the activity. The participant (or parent/guardian) accepts personal financial responsibility for any injury or other loss sustained during the activity or during transportation to and from the activity, as well as for any medical treatment rendered to the participant that is authorized by the sponsor or its agents, employees, volunteers, or any other representatives (collectively referred to as the “activity sponsor”). Further, the participant (or parent/guardian) releases and promises to indemnify, defend, and hold harmless the activity sponsor for any injury arising directly or indirectly out of the described activity or transportation to and from the activity, whether such injury arises out of the negligence of the activity sponsor, the participant, or otherwise.
If a dispute over this agreement or any claim for damages arises, the participant (or parent/guardian) agrees to resolve the matter through a mutually acceptable alternative dispute resolution process. If the participant (or parent/guardian) and the activity sponsor cannot agree upon such a process, the dispute will be submitted to a three-member arbitration panel for resolution in accordance with the rules of the American Arbitration Association.
