Summer Camp Waiver & Registration Confirmation
I understand that participating in True Taekwondo Summer Camp involves physical activity, martial arts, sports, and field trips, and that injuries or accidents may occur.

I certify that my child is physically able to participate. In the event of an emergency, I authorize True Taekwondo staff to seek medical treatment for my child and understand that I am responsible for any associated costs.

I release and hold harmless True Taekwondo LLC, its instructors, staff, and volunteers from any claims, injuries, or damages arising out of or related to participation in the Summer Camp Program.

I consent to photos and videos of my child being taken during camp for instructional, promotional, social media, or marketing purposes.

I understand that submission of this registration form does not guarantee a spot in the Summer Camp Program. Registration is only considered complete once the required deposit, completed Summer Camp Agreement, and waiver forms have been received and approved by True Taekwondo LLC.

Camp spaces are limited and will be secured on a first-come, first-served basis once all required documents and payments have been received.

True Taekwondo LLC reserves the right to decline or cancel registration if required forms or payments are not submitted.
