Registered Nurse (RN) – Home Care
Position Summary
The Registered Nurse (RN) provides skilled nursing care to patients in their homes, ensuring high-quality, individualized, and compassionate care. This role focuses on promoting patient independence, managing chronic conditions, and preventing hospital readmissions through effective care planning and education.
Key Responsibilities
1. In-Home Patient Care
Conduct comprehensive in-home assessments of patients’ physical, emotional, and environmental needs. Develop, implement, and update individualized care plans. Monitor patient conditions and report changes to physicians
2. Skilled Nursing Services
Administer medications, injections, and IV therapies as ordered. Perform wound care, catheter care, and other clinical procedures. Manage chronic conditions such as diabetes, hypertension, and COPD
3. Care Coordination
Collaborate with physicians, therapists, home health aides, and family members. Coordinate services to ensure continuity of care. Participate in case conferences and care reviews
4. Patient & Caregiver Education
Educate patients and families on disease management and medication use. Provide guidance on diet, mobility, and safety in the home. Teach proper use of medical equipment
5. Documentation & Compliance
Maintain accurate and timely documentation in electronic health records (EHR). Ensure compliance with state regulations, Medicare/Medicaid guidelines, and agency policies
6. Safety & Quality Assurance
Assess home environments for safety risks and recommend modifications. Follow infection control protocols and safety standards. Support efforts to reduce hospitalizations and improve patient outcomes
Licensed Practical Nurse (LPN)
Key Responsibilities:
1.Provide direct patient care, including vital signs monitoring, medication administration, wound care, and basic clinical procedures.
2. Observe and document patient conditions, reporting changes promptly to supervising RNs or physicians.
3. Assist with patient hygiene, mobility, and daily living activities as needed.
4. Educate patients and families on health management, medications, and preventive care.
5. Collaborate with the healthcare team to implement and update patient care plans.
6. Maintain accurate patient records in compliance with organizational and regulatory standards.
7. Adhere to infection control policies and ensure a safe and clean environment for patients.
8. Respond to patient needs and emergencies promptly and efficiently.
