Acconsente al trattamento e alla pubblicazione, per i soli fini istituzionali, di video, fotografie e/o immagini atte a rivelare l’identità del minore, sul sito web di Kenkoo Asd e Aikido Shion Asd e relativi social. Il sottoscritto, consapevole delle sanzioni penali, in caso di dichiarazioni non veritiere, AFFERMA la idoneità del minore a partecipare a questa attività sportiva, sollevando gli organizzatori da qualsiasi responsabilità per eventuali problemi fisici e/o medici che potrebbero sorgere. Dichiara inoltre che il minore è regolarmente assicurato per la pratica dell'Aikido attraverso l'organizzazione a cui appartiene.

INFORMATIVA PRIVACY AI SENSI ART. 13 D. LGS 196/2003

La informiamo con la presente che il Decreto Legislativo n. 196 del 30 giugno 2003 (“Codice in materia di protezione dei dati personali”) prevede la tutela delle persone e di altri soggetti rispetto al trattamento dei dati personali. Ai sensi della predetta normativa, il trattamento dei Suoi dati personali sarà improntato ai principi di correttezza, liceità e trasparenza e di tutela della Sua riservatezza e dei Suoi diritti. Ai sensi dell’art. 13 del Decreto Legislativo n. 196 del 30 giugno 2003, Le forniamo le seguenti informazioni:

1. Finalità e modalità del trattamento dei dati. I Suoi dati personali ci sono stati forniti e saranno trattati esclusivamente per finalità strettamente connesse e strumentali all’adempimento delle obbligazioni inerenti ai rapporti con la Ns. Società in particolare:

- per l’inserimento delle anagrafiche nei data base informatici aziendali; - per la tenuta della contabilità; - per la gestione degli incassi e pagamenti; - per adempiere agli obblighi previsti dalle norme di legge civilistiche e fiscali, dai regolamenti, dalla normativa comunitaria. Il trattamento dei dati personali sarà effettuato tramite supporti cartacei ed informatici dal titolare, dal responsabile/dai responsabili e dagli incaricati con l’osservanza di ogni misura cautelativa, che ne garantisca la sicurezza e riservatezza.

2. Natura della raccolta dei dati e conseguenze di un eventuale mancato conferimento

Il conferimento dei Suoi dati personali è obbligatorio al fine di adempiere agli obblighi derivanti dal contratto e, in generale, agli adempimenti di legge. Il loro eventuale mancato conferimento potrebbe comportare l’impossibilità da parte nostra di adempiere agli obblighi contrattuali.

3. Comunicazione e diffusione dei dati

I Suoi dati personali, ai fini dell’esecuzione del contratto e per le finalità sopra indicate, potranno essere comunicati:3.1) a tutte le persone fisiche e giuridiche (studi di consulenza legale, amministrativa, fiscale, società di revisione, corrieri e spedizionieri, centro elaborazione dati etc.) nei casi in cui la comunicazione risulti necessaria per le finalità sopra illustrate; 3.2)ad istituti bancari per la gestione degli incassi e dei pagamenti; 3.3) a società di factoring o di recupero crediti; 3.4) ai nostri collaboratori e dipendenti appositamente incaricati e nell’ambito delle relative mansioni;

4. Diritti dell’interessato

In ogni momento potrà esercitare i Suoi diritti nei confronti del titolare del trattamento ai sensi dell’art. 7 del Decreto Legislativo n. 196 del 30 giugno 2003, che per Sua comodità riproduciamo. 4.1) L'interessato ha diritto di ottenere la conferma dell'esistenza o meno di dati personali che lo riguardano, anche se non ancora registrati, e la loro comunicazione in forma intelligibile. 4.2). L'interessato ha diritto di ottenere l'indicazione: 4.2a) dell'origine dei dati personali; 4.2b) delle finalità e modalità del trattamento; 4.2c) della logica applicata in caso di trattamento effettuato con l'ausilio di strumenti elettronici; 4.2d) degli estremi identificativi del titolare, dei responsabili e del rappresentante designato ai sensi dell’articolo 5, comma 2; 4.2e) dei soggetti o delle categorie di soggetti ai quali i dati personali possono essere comunicati o che possono venirne a conoscenza in qualità di rappresentante designato nel territorio dello Stato, di responsabili o incaricati. 4.3) L'interessato ha diritto di ottenere: 4.3a) l'aggiornamento, la rettificazione ovvero, quando vi ha interesse, l'integrazione dei dati; 4.3b) a cancellazione, la trasformazione in forma anonima o il blocco dei dati trattati in violazione di legge, compresi quelli di cui non è necessaria la conservazione in relazione agli scopi per i quali i dati sono stati raccolti o successivamente trattati; 4.3c) l'attestazione che le operazioni di cui alle lettere a) e b) sono state portate a conoscenza, anche per quanto riguarda il loro contenuto, di coloro ai quali i dati sono stati comunicati o diffusi, eccettuato il caso in cui tale adempimento si rivela impossibile o comporta un impiego di mezzi manifestamente sproporzionato rispetto al diritto tutelato. 4.4. L'interessato ha diritto di opporsi, in tutto o in parte: 4.4a) per motivi legittimi al trattamento dei dati personali che lo riguardano, ancorché pertinenti allo scopo della raccolta; 4.4b) al trattamento di dati personali che lo riguardano a fini di invio di materiale pubblicitario o di vendita diretta o per il compimento di ricerche di mercato o di comunicazione commerciale.

I diritti di cui all'articolo 4 sono esercitati con richiesta rivolta senza formalità al titolare o al responsabile, anche per il tramite di un incaricato, alla quale è fornito idoneo riscontro senza ritardo. La richiesta rivolta al titolare o al responsabile può essere trasmessa anche mediante lettera raccomandata, telefax o posta elettronica.

PRIVACY POLICY PURSUANT TO ARTICLE 13 OF LEGISLATIVE DECREE 196/2003

We hereby inform you that Legislative Decree no. 196 of 30 June 2003 ("Personal Data Protection Code") provides forthe protection of persons and other subjects with regard to the processing of personal data.

Pursuant to the aforementioned legislation, the processing of your personal data will be based on the principles of correctness, lawfulness and transparency and the protection of your privacy and your rights. Pursuant to art. 13 of Legislative Decree no. 196 of 30 June 2003, we provide you with the following information: 

1. Purposes and methods of data processing. Your personal data have been provided to us and will be processed exclusively for purposes strictly connected and instrumental to the fulfilment of the obligations inherent in relations with our Company, in particular: - for the insertion of personal data in the company's computer databases; - for bookkeeping; - for the management of collections and payments; - to fulfil the obligations provided for by civil and tax law, regulations, EU legislation. The processing of personal data will be carried out on paper and computer by the data controller, the manager(s) and the persons in charge with the observance of all precautionary measures, which guarantee their security and confidentiality.

2. Nature of data collection and consequences of failure to provide it The provision of your personal data is mandatory in order to fulfil the obligations arising from the contract and, in general, the fulfilment of legal obligations. Failure to provide them may make it impossible for us to fulfil our contractual obligations.

3. Communication and dissemination of data- Your personal data, for the purposes of executing the contract and for the purposes indicated above, may be communicated: - to all natural and legal persons (legal, administrative, tax consultancy firms, auditing firms, couriers and freight forwarders, data processing centres, etc.) in cases where communication is necessary for the purposes illustrated above; - to banking institutions for the management of collections and payments; - to factoring or debt collection companies; to our collaborators and employees specifically appointed and within the scope of their duties; 4. Rights of the data subject
At any time you may exercise your rights towards the data controller pursuant to art. 7 of Legislative Decree no. 196 of 30 June 2003, which we reproduce for your convenience. 3.1. The data subject has the right to obtain confirmation of the existence or otherwise of personal data concerning him/her, even if not yet recorded, and their communication in intelligible form. 3.2. The interested party has the right to obtain information:3.2a) the origin of the personal data; 3.2b) the purposes and methods of processing; 3.2c) the logic applied in the case of processing carried out with the aid of electronic tools; 3.2d) the identification details of the data controller, the data processors and the representative designated pursuant to Article 5, paragraph 2; 3.2e) of the subjects or categories of subjects to whom the personal data may be communicated or who may become aware of them in their capacity as designated representatives in the territory of the State, data processors or persons in charge. 3.3. The data subject has the right to obtain: 3.3.a) updating, rectification or, when interested, integration of data; 3.3.b) the cancellation, transformation into anonymous form or blocking of data processed in violation of the law, including those whose retention is not necessary in relation to the purposes for which the data were collected or subsequently processed; 3.3c) certification that the operations referred to in letters a) and b) have been brought to the attention, also as regards their content, of those to whom the data have been communicated or disseminated, except in the case in which this fulfilment proves impossible or involves the use of means manifestly disproportionate to the protected right.

4. The data subject has the right to object, in whole or in part: 4.a) for legitimate reasons to the processing of personal data concerning him/her, even if pertinent to the purpose of the collection; 4.b) to the processing of personal data concerning him/her for the purpose of sending advertising material or direct sales or for carrying out market research or commercial communication. The rights referred to in Article 7 shall be exercised by means of an informal request addressed to the data controller or processor, also through a person in charge, to which appropriate response shall be provided without delay. The request addressed to the data controller or the data processor may also be sent by registered letter, fax or e-mail.
