At Heart of Florida Gators, we truly believe every child deserves the opportunity to play, learn, and grow. While we wish we could offer scholarships to every family in need, our available funding is limited and cannot be guaranteed.

Please know that every request is reviewed thoughtfully and with fairness. We are always working behind the scenes to secure additional resources so we can support as many of our youth as possible.

All families requesting scholarship assistance are required to complete the Heart of Florida Gators Scholarship Request Form for consideration.

I, the parent/guardian of the above-named participant, understand that participation in youth football, flag football, conditioning, practices, games, travel, and related activities involves inherent risks, including but not limited to falls, collisions, physical contact, equipment-related injuries, weather-related risks, and other serious injury risks.

I voluntarily allow my child to participate in activities associated with Heart of Florida Gators Football & Tutoring Foundation and knowingly assume all risks connected with such participation, whether known or unknown.

To the fullest extent permitted by law, I release, waive, and discharge Heart of Florida Gators Football & Tutoring Foundation, its board members, officers, directors, coaches, team parents, volunteers, sponsors, partners, and affiliates from any and all claims, liabilities, demands, actions, damages, costs, or causes of action arising out of or related to my child's participation, except in cases of gross negligence or intentional misconduct.

In the event of an accident, illness, or emergency involving my child, I authorize Heart of Florida Gators Football & Tutoring Foundation staff, coaches, volunteers, or representatives to obtain emergency medical care for my child if I cannot be reached immediately.

I understand reasonable efforts will be made to contact me first. I acknowledge that I am financially responsible for any medical treatment, transportation, or related costs incurred on behalf of my child. I certify that the medical information provided in this registration is accurate to the best of my knowledge and that my child is physically able to participate unless otherwise disclosed in writing.

I grant permission to Heart of Florida Gators Football & Tutoring Foundation to photograph, video record, or otherwise capture the likeness of my child during practices, games, events, camps, fundraisers, and community activities.

I understand these images and recordings may be used for lawful promotional purposes, including social media, websites, flyers, banners, sponsorship materials, and other marketing or educational communications, without compensation to me or my child.

PARENT & PLAYER CODE OF CONDUCT AGREEMENT
By registering, we agree to support a safe, respectful, and positive environment for all participants. We will demonstrate good sportsmanship, respect coaches, officials, teammates, opponents, families, and staff, and follow all organization, field, and league rules.

We understand that bullying, harassment, fighting, abusive language, threatening behavior, repeated misconduct, or actions that disrupt the mission of the organization may result in disciplinary action, including suspension or removal from the program without refund, when permitted by policy.

At Heart of Florida Gators, we believe academics come first. Participation in our program is a privilege that includes a commitment to effort and accountability in the classroom.

While specific academic standards may be updated throughout the season, all players are expected to demonstrate consistent effort, positive behavior, and a commitment to their schoolwork.

Players who are not meeting academic expectations, as determined by the organization, may be required to participate in mandatory academic support. This includes attending a minimum of two (2) tutoring sessions per week until improvement is shown.

Our goal is not to penalize, but to support and guide each student-athlete toward success both on the field and in the classroom
