I acknowledge that participation in art activities carries inherent risks (cuts, scrapes, paint stains, allergic reactions, etc.). I release and hold harmless Little Studio Summers, its owners, instructors, assistants, employees, and volunteers from any and all liability for injury, loss, or damage to personal property arising from participation in camp activities, except in cases of gross negligence or willful misconduct.
I certify the health, allergy, and medication information provided is complete and accurate. I authorize camp staff to provide minor first aid (bandages, cleaning wounds, ice packs).
I grant Studio Sprouts permission to use photographs and/or video of my child taken during camp for promotional, educational, and social media purposes. No names will be used without separate consent.
If my child shows symptoms of an infectious illness before or during camp, I will keep them home and follow the camp’s illness policy. Refunds follow the stated cancellation policy.
