Waiver, Release of Liability, Medical Consent, and Parent Agreement Participant Information
1. Acknowledgment of Risk I, the parent/guardian of the above-named child (“Participant”), understand that participation in Mission Makers programs includes activities that may take place both indoors and outdoors, including but not limited to: Movement-based games and challenges, obstacle-style activities, team-based problem-solving tasks, use of equipment such as cups, dominos, balls, and other materials, activities conducted on indoor floors, outdoor surfaces, and open spaces. I understand that these activities involve inherent risks, including but not limited to: Slips, trips, and falls, collisions with objects or other participants, minor injuries such as cuts, bruises, or sprains. I voluntarily assume all risks, both known and unknown, associated with my child’s participation.
2. Release of Liability. To the fullest extent permitted by law, I hereby release, waive, and discharge: Mission Makers (including its owner, operators, and affiliates) All employees, assistants, volunteers, and helpers (both paid and unpaid) The church and/or facility where the program is held from any and all liability, claims, demands, or causes of action arising out of or related to any injury, illness, damage, or loss that may occur as a result of my child’s participation in the program. This includes claims arising from ordinary negligence, but does not apply to gross negligence or willful misconduct.
3. Medical Authorization and Emergency Care I certify that my child is physically able to participate in camp activities. I agree to: Provide accurate medical, allergy, and emergency contact information, inform Mission Makers of any conditions that may impact participation. In the event of illness or injury, I authorize Mission Makers staff to: Administer basic first aid, seek emergency medical treatment if necessary. I understand that I am financially responsible for any medical care provided.
4. Behavioral Expectations and Dismissal Policy. Mission Makers is a structured program designed to support children in developing executive functioning, self-regulation, and social skills. For the safety of all participants, I understand and agree that: My child must be able to participate in a group setting safely and appropriately, including: Following directions, remaining with the group, engaging without causing harm to others or the environment. The following behaviors are not permitted: Physical aggression toward others, destruction of property, repeated refusal to follow safety directions, elopement (leaving the group or designated area without permission) Any behavior that compromises the safety of the child or others. Mission Makers reserves the right to: Contact guardian and/or emergency contacts listed above for early pick-up if needed, dismiss my child from the program if behaviors are unsafe or significantly disruptive, dismissal due to safety or behavioral concerns may occur without refund.
5. Use of Facilities. I understand that Mission Makers operates within a rented facility and may use both indoor and outdoor areas. I agree that: The church/facility is not responsible for supervision or programming. My child must follow all facility rules and expectations. Any damage caused by my child may be my financial responsibility
6. Photo and Media Release. I grant permission for my child to be photographed or recorded during participation in Mission Makers activities. I understand these images may be used for: Website content, Social media (including Instagram and Facebook) Marketing materials and internal use during camp learning. No identifying personal information (such as full name) will be shared without additional consent.
7. Educational Program Disclaimer (Non-Medical / Non-Therapeutic Services)
Mission Makers is an educational enrichment program designed to support the development of executive functioning, self-regulation, and problem-solving skills through structured activities and guided experiences. I understand that Mission Makers is not a medical, psychological, or therapeutic service and does not provide diagnosis, treatment, or therapy for any medical or mental health condition, including ADHD or related challenges. I acknowledge that participation in Mission Makers is not a substitute for medical care, psychological services, or professional therapy. I agree that any concerns regarding my child’s medical, behavioral, or emotional needs should be addressed with a qualified healthcare or mental health professional.
8. Agreement and Acknowledgment, By signing below (or checking the agreement box during online registration), I confirm that: I have read and fully understand this Waiver and Release of Liability. I understand the risks involved in participation. I voluntarily agree to all terms stated above. I am the legal parent or guardian of the participant.
