Sonoma Community Tennis Program Photo & Media Release

By clicking “I Agree,” you confirm that you are the parent or legal guardian of the participating child and grant permission to Sonoma Community Tennis, its staff, coaches, volunteers, and affiliated organizations to photograph, videotape, and/or record your child during program activities. You acknowledge and agree that:

• Your child’s image, video, or likeness may be used for program-related purposes, including website content, social media, newsletters, marketing materials, and local media coverage.
• These materials may be used without compensation or further notice.
• Your child’s first name may be used in connection with images unless you notify the program otherwise in writing.
• All media will be used in a respectful and appropriate manner consistent with the mission of the program.

You further agree to release and hold harmless Sonoma Community Tennis, its representatives, and affiliates from any claims, liabilities, or damages arising from the use of such images or recordings. You may revoke this consent at any time by submitting written notice to Felipe Monroy, but previously published materials cannot be withdrawn.

Sonoma Community Tennis Program Transportation Release

By clicking “I Agree,” you confirm that you are the parent or legal guardian of the participating child and grant permission for your child to be transported in a vehicle operated by Sonoma Community Tennis, including program-owned or authorized vans and privately-owned vehicles, for purposes related to participation in program activities. You acknowledge and agree that:

• Your child may be transported by approved staff, coaches, or authorized drivers designated by Sonoma Community Tennis.
• Transportation may include travel to and from program sites, matches, events, or related activities.
• Participation in transportation involves inherent risks, including but not limited to vehicle accidents, road conditions, weather conditions, and other unforeseen circumstances.
• Your child is expected to follow all safety rules and instructions while in the vehicle, including wearing a seatbelt at all times in accordance with California law.

You authorize Sonoma Community Tennis staff to obtain emergency medical care for your child if necessary during transportation, and you agree to be responsible for any resulting medical expenses.

Release of Liability: TO THE FULLEST EXTENT PERMITTED UNDER CALIFORNIA LAW, YOU, ON BEHALF OF YOURSELF AND YOUR CHILD, AGREE TO RELEASE, WAIVE, AND DISCHARGE SONOMA COMMUNITY TENNIS, ITS DIRECTORS, OFFICERS, EMPLOYEES, AGENTS, VOLUNTEERS, AND AFFILIATED ORGANIZATIONS FROM ANY AND ALL CLAIMS, DEMANDS, CAUSES OF ACTION, OR LIABILITY ARISING OUT OF OR RELATED TO YOUR CHILD’S TRANSPORTATION, INCLUDING THOSE ARISING FROM THE ORDINARY NEGLIGENCE OF THE RELEASED PARTIES. This release does not apply to claims arising from gross negligence, reckless conduct, or willful misconduct.

You understand that you may revoke this consent at any time by submitting written notice to Felipe Monroy, but that revocation may affect your child’s ability to participate in activities requiring transportation.
