I acknowledge that I am the Legal Parent/Guardian of the child I am registering for BLW. I give approval for their participation in the league. I assume all risks incidental to the conduct of the activities and transportation to and from the activities. We release, absolve, and hold harmless the BLW and Brookfield local Schools, the organizers, sponsors, supervisors, and anyone connected with the league. In case of injury to my child, I hereby waive all claims against the organizers and supervisors of the league. I also give permission to the BLW youth program to publish and reproduce any photographs PR images of my child/children in print or video form. As a member of BLW Cheer, I agree to be responsible for returning my team issued uniform. All uniforms must be returned clean and in good repair at the end of the 2025 season. I will return my uniform on the designated day set by the cheer advisor in a bag that is sealed with the player's name on the outside. If I fail to return the uniform to the cheer advisor on the selected day, or if I return the uniform damaged, I will be held responsible for a replacement or cleaning fee.

A replacement fee is as follows:
Cheer shell (top) $ 50
Cheer skirt $50
Cheer liner $35
A cleaning fee is as follows:
$50

If I fail to return the uniform or pay the replacement/ cleaning fee, players will be ineligible to participate in the following season.

Information on how to care for cheer uniforms will be sent to the email parents/ guardians list for registration.

Cost to cheer: $100

Payments may be made via Venmo, cash, or check. Please make checks payable to "Brookfield Little Warriors" or "BLW". If paying via Venmo please put your player's name in the memo.
