(the undersigned) hereby authorize International Dog Sports, LLC (the “organization”) and its partners, agents, representatives, assigns, successors in interest and licensees, to photograph, audiotape, and/or videotape me and grant the Organization and its partners the irrevocable right to use my photograph, audio recording, video recording, or any reproduction or modification thereof (the “Photograph,” “Audio,” and/or “Video”), in any manner or medium throughout the world an unlimited number of times in perpetuity in advertising, trade, promotion, exhibition, or any other lawful purpose. I understand that I will not receive any monetary compensation for the permissions I am granting herein. I hereby waive any right of inspection of approval of the uses to which the Organization may put the Photograph, Audio, and/or Video. I acknowledge the Organization will rely on this permission and hereby release and discharge the Organization from any and all claims and demands arising out of or in connection with the Photograph or the exercise of the permissions granted here, including any or all claims for libel, invasion of privacy, or emotional distress. I understand that I cannot withdraw my consent after I sign this form and that this consent and release is binding on me and my heirs, legal representatives and assigns. I grant permission for Photographs to be collected and used by the Organization. I grant permission for Audio to be collected and used by the Organization. I grant permission for Video to be collected and used by the Organization. GENERAL RELEASE OF LIABILITY In consideration of being permitted to participate in an International Dog Sports, LLC. event (“Event”), the above signed Owner releases, waives, discharges, and covenants not to sue International Dog Sports, LLC., including its owners, members, directors, employees, and volunteers for any damage to their person or property resulting from any accident or incident at the event. The Owner agrees that the facility holding the Event has the right to refuse entry for any cause which the facility shall deem sufficient. Assumption of Risk: Participation in the Event carries with it certain inherent risks that cannot be eliminated regardless of the care taken to avoid injuries. The specific risks vary from one Event to another, but the risks range from 1) minor injuries such as scratches, bruises, and sprains; 2) major injuries such as eye injury or loss of sight, joint or back injuries, heart attacks, and concussions; 3) catastrophic injuries including paralysis and death, and 4) injury to personal property including their pet. I have read the previous paragraphs and I know, understand, and appreciate these and other risks that are inherent in the Event. I hereby assert that my participation is voluntary and that I knowingly assume all such risks. Indemnification and Hold Harmless: I also agree to INDEMNIFY AND HOLD International Dog Sports, LLC. HARMLESS from all claims, actions, suits, procedures, costs, expenses, damages, and liabilities, including attorney’s fees brought as a result of my involvement in the Event and to reimburse them for any such expenses incurred. Severability: The undersigned further expressly agrees that the foregoing release is intended to be as broad and inclusive as is permitted by the law of the State of Ohio and that if any portion thereof is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect. International Dog Sports, LLC. is not responsible for any errors on the entry form or any errors made by International Dog Sports, LLC. Photos: I grant International Dog Sports, LLC., and its affiliated companies, representatives, sponsors, the right to license, reproduce, distribute, and display all photographs, videos, and digital images of myself and my dog at the Event. Acknowledgment of Understanding: I have read this waiver of liability, assumption of risk, and indemnity agreement, fully understand its terms, and understand that I am giving up substantial rights, including my right to sue. I acknowledge that I am signing the agreement freely and voluntarily and intend by my signature to be a complete and unconditional release of all liability to the greatest extent allowed by law. I acknowledge that the terms of this contract are a complete and final agreement between myself and International Dog Sports, LLC. and is to be construed under Ohio law.

Choose OPTION #1 to PAY IN FULL for expedited check in the day of the event OR Choose OPTION #2 to only pay the minimum required deposit (20% of the total) to reserve your rounds and pay the balance at the time of check in. All payments are non-refundable without a Vet or Doctor's note for the dog or handler listed. A 3.5% processing fee has been added to the price.
