    Please fill out this intake form as thoroughly as possible so I can learn more about you and your fur baby before your appointment. I also kindly ask that you send over a current photo of your pet along with your form submission so I can better assess coat condition, size, and grooming needs prior to the appointment. All inquiries will receive a response within 24 hours so we can discuss the next steps.

  • HEALTH & SAFETY POLICY - For the health & safety of your pet, I do not groom pets with open wounds, active bleeding, untreated skin infections, or contagious medical conditions. If an open wound, hot spot, or untreated skin issue is discovered at the time of the appointment, services may be rescheduled until the condition has been properly treated and healed. Please confirm your understanding below: *
  • BEHAVIORAL & AGGRESSION POLICY - The safety of your pet and groomer is top priority. While I understand some pets may feel nervous during grooming, I do reserve the right to refuse, stop, or modify services at any time if a pet displays aggressive behavior, excessive stress, or poses a safety risk. Additional handling fees may apply for pets requiring extra time, special handling, or safety precautions. Please confirm your understanding below:*
  • MATTING POLICY - In cases of severe matting, shaving may be the most humane and safest option for your pet's comfort and well-being. Severe matting can potentially cause irritation, sores, redness, or pre-existing skin conditions that may become visible once the coat is removed. I will not be responsible for any skin conditions, irritations, or medical issues that are revealed as a result of removing a severly matted coat. Additional fees may apply for excessive matting due to the extra time and labor. Please confirm your understanding below:*
  • FLEA & TICK POLICY - The health and safety of all pets and the grooming environment is extremely important. If live fleas are discovered at the time of your pet's appointment, a mandatory $10 flea treatment fee will be added to your service total. This covers the cost of specialized flea shampoo & the necessary sanitation & treatment of the grooming area to prevent contamination and protect future clients. If a severe infestation is present, the groomer reserves the right to stop or refuse service and may recommend veterinary treatment. Please confirm your understanding below:*
  • PHOTO & SOCIAL MEDIA RELEASE - I may photograph pets before, during, or after their grooming appointment for portfolio.Photos may be used on social media platforms to showcase my work. No personal client informational will ever be shared.
