Any athlete on the U.S. World Team and Nations Cup Team must submit a responsible person form. A responsible person must be a participant with the U.S. Tour and purchase the designated U.S. Tour Package.

Every athlete (regardless of age) on the U.S. Tour MUST list a responsible person. The Responsible Person listed must also be on the U.S. Tour as a spectator or primary coach. If the athlete is under 18 years old, a parent's signature is required.
I fully understand and accept all responsibilities, including financial responsibilities, placed upon me concerning my son/daughter's position as a member of the IBTF World Freestyle and Rhythmic Championships and/or Nations Cup A Level Team, and I give my complete support to my son/daughter's decision to accompany the U.S. World/Nations Cup Team to compete in the IBTF World Championships.
