ALPHA PNO WAIVER AGREEMENT
IMPORTANT PNO POLICIES
1. Program Hours: I understand PNO hours are 6:00 PM–9:30 PM and I agree to pick up my child on time.
2. No Adults in Gym Area: For safety and focus, no adults are allowed inside the gym activity area during PNO.
3. Check-In/Out: I will check my child in and out at the front desk and present ID if requested.
3. Late Pick-Up Fee: I understand that pick-up after 9:30 PM will result in a $5 late fee applies at 6 minutes past the scheduled time, with an additional $5 fee charged for every subsequent 5-minute increment.
3. Food: Dinner price $8 per child. Includes 2 slices of Pizza, 2 snacks and a drink. Kids must bring large bottles of water.
4. Behavior: My child must follow coach instructions and safety rules. Alpha may remove a child from activities for unsafe behavior; parent/guardian will be contacted for immediate pickup.
RELEASE OF LIABILITY & ASSUMPTION OF RISK (READ CAREFULLY)
I, the undersigned parent/legal guardian, understand that participation in gymnastics, tumbling, open gym, inflatable/bounce house activities, games, and related physical activities involves inherent risks, including but not limited to falls, collisions, sprains, strains, fractures, illness, and other injuries, which may occur even with careful supervision and proper equipment.
In consideration of my child being permitted to participate, I voluntarily assume all risks associated with participation and agree as follows:
1. Assumption of Risk. I knowingly and voluntarily assume all risks of injury, loss, or damage arising from or related to my child’s participation in PNO activities, including the use of equipment and facilities.
2. Release & Waive. To the fullest extent permitted by Florida law, I hereby release, waive, discharge, and hold harmless Alpha Gymnastics & Recreation, Alpha Gymnastics LLC, Alpha Recreation, its owners, officers, directors, employees, contractors, coaches, volunteers, landlords, and agents (“Released Parties”) from any and all claims, demands, causes of action, damages, costs, or liability of any kind arising out of or related to my child’s participation, including claims based on ordinary negligence of the Released Parties.
3. Indemnification. I agree to indemnify and defend the Released Parties from any claim brought by or on behalf of my child or any third party arising out of my child’s participation, to the fullest extent permitted by law.
