I, the undersigned parent/guardian of the participant listed above, acknowledge that participation in soccer and related activities involves inherent risks, including but not limited to injury, illness, permanent disability, or death.

I voluntarily assume all risks associated with my child’s participation in Burlington United FC Youth tryouts, training sessions, and related activities.

I hereby release, waive, and discharge Burlington United FC, its directors, staff, coaches, volunteers, and representatives, as well as Burlington Parks and Recreation, facility providers, field owners, event organizers, sponsors, and any other individuals or organizations involved in the operation of these activities, from any and all liability, claims, demands, or causes of action arising out of or related to any injury, illness, or damages that may occur as a result of participation.

I consent to my child receiving emergency medical treatment if necessary and understand that I am responsible for any associated medical expenses.

I certify that my child is physically fit to participate in athletic activities and has no medical conditions that would prevent safe participation unless disclosed in this form.
