  Thank you for choosing Islamic School of MOMIN for your child(ren)'s Islamic education. We serve students between 5 to 15 years old across grades 1-10. Our young adults between ages 16-18 are enrolled in special discussion based Continued Education program that has it's separate registration (link).
  
  Here is some important information to review before you begin the application. Please note that registration is not complete until payment has been submitted at the end of application. You will receive a confirmation email upon successful submission.
  
  Time & Dates
  - First day of school will be Aug 16th, 2026 Insha'Allah
  - Last day to register without late fee penalty is July 31st, 2026
  - No registrations will be accepted after Aug 15th, 2026 (or before if class limits are reached)
  - School starts at 10:30AM and ends at 2:15PM
  School Fees
  - These fees help us cover books, school supplies and special events such as field trip, picnic, Parents' Day, etc.
  - Early bird until May 31st, 2026 - $300/student, capped at $475 per family
  - Regular fee starting June 1st, 2026 - $375/student, capped at $675 per family
  - MOMIN members get $50 off regular fee - $325 per student, capped at $625 per family. Must be active paid member to receive this discount.
  - ISM teachers and staff get additional discount - $275 per student, capped at $350 per family. Must be active member of staff in teaching or volunteer with 90% participation in Sunday school to receive this discount.
  - Discounts cannot be combined
  - Late registration fee of $25 applies starting Aug 1st, 2026. No discounts will be honored for late registration
  - Financial assistance is available to those who qualify
    
    $25 registration fee per student is applicable

  Lunch
  
  Students have an option to have lunch provided by ISM. Lunch includes a slize of pizza, juice and chips or a different snack. The cost is $120/student for the year. If you'd like extra large lunch (2 slices instead of 1), it is an additional $60/student.

- Liability Waiver: The submitter of this form, who is a legal guardian of the above student(s), agrees to completely indemnify and forever hold harmless all the Teachers, Administrators, Executive Committee members, and the General Body of the Islamic Center of MOMIN and its Islamic School from all liabilities that may result from an accident or injury be it physical or mental, to the student as a result of attending the Islamic School of MOMIN.
