Liability, Consent and Acknowledgement

In consideration of being allowed the privilege of participating as a volunteer with LIYFC, I hereby release LIYFC, YFC/USA, and its employees, officers, directors, ministry centers, chapters, affiliates and all other affiliated persons from and against any and all liability, claims, and damages that I may have now or in the future arising in any way from my participation as a volunteer with Youth for Christ. I also agree to indemnify, defend and hold harmless LIYFC & YFC/USA from and against any and all claims, demands, actions and judgments from other persons claiming by or through me due to my participation as a volunteer for Youth for Christ. I affirm that I am 18 years of age or older and represent that I have my own medical and other insurance coverage to provide me benefits and protections in the event that I am injured or if I cause liability to others.

I further represent the following:
1) I have never been accused or convicted of, any offense of sexual misconduct, child abuse, injury to a child, or indecency with a child.
2) I have never been convicted of an offense involving violence or physical harm against another person.
3) As of this date I do not have any pending criminal charges against me.

I hereby grant permission to Youth for Christ the right to use, reproduce, and/or distribute any photographs, films, videos and sound recording of me, without compensation or approval right, for use in materials created for purpose of promoting the future activities of Youth for Christ.
