- I, the parent or legal guardian of the above-named athlete, hereby give permission for my child to participate in the SACK U Elite Big Man OL/DL 8 WEEK PROGRAM powered by Protostar Prep Academy.
- I acknowledge that participation in athletic activities involves inherent risks, including but not limited to physical injury. I voluntarily assume all risks associated with participation.
- I hereby release, waive, and discharge Protostar Prep Academy, its coaches, staff, volunteers, partners, and facility providers from any and all claims, liabilities, or causes of action arising out of participation in this event.
- I certify that my child is physically able to participate and has no medical condition that would prevent safe participation.
- I grant permission for emergency medical treatment if necessary.
- I also grant permission for photos or videos taken during the camp to be used for promotional and educational purposes.
