    Our records indicate that you are a participant in at least one of the Funds named above. The purpose of this email is to provide you with an option to opt in to paperless delivery of documents as allowed under law.  If you chose to not opt in you will continue to receive all Funds communications through U.S. Postal Service.

  • 1. Documents Covered by This Consent

  • If you provide your consent, the Funds may furnish to you electronically any documents that the
    Plan is required to provide, including, but not limited to:

    • Summary Plan Descriptions
    • Summaries of Material Modifications
    • Summary Annual Reports
    • Notices required under HIPAA
    • Benefit statements and other disclosures required by ERISA

  • 2. Method of Electronic Delivery

  • If you consent, documents will be provided to you by one or more of the following methods:
    • Email sent to your designated email address or
    • Posting on a website, with notice to you of the availability of the document.

  • 3. Hardware and Software Requirements

    • To access documents electronically, you will need:
      A computer, smartphone, or similar device with internet access,
    • A valid and active email address,
    • A web browser capable of viewing HTML and PDF documents (such as Adobe Reader or similar software), and
    • Sufficient storage or printing capability to retain documents for your records.

  • 4. Your Right to Receive Paper Copies

  • You have the right to request and obtain a paper copy of any document furnished electronically, free of charge. 

  • 5. Your Right to Revoke Consent

  • You may revoke your consent to receive documents electronically at any time without charge. Revocation of consent will apply to future documents and will not affect the validity of any electronic delivery made prior to your revocation.

  • It is your responsibility to provide the Fund Office with a current and valid email address and to promptly notify it of any changes. Failure to do so may result in your not receiving important Fund information.

  • 7. Voluntary Consent

  • Your consent to receive documents electronically is completely voluntary. You are not required to consent. If you do not consent, you will continue to receive paper copies of required documents.
