Please register to attend our pre-season gym workouts. Included are the waivers necessary for participation. This form is for parents/guardians of athletes ages 17 & under; athletes 18+ may register on their own.

Further, I attest I am the parent or legal guardian of the athlete and have full authority to consent and authorize the Organization to use their likenesses and names. I further acknowledge participation is voluntary and neither I nor the athlete will receive financial compensation of any type associated with the taking or publication of these photographs or participation in the Organization's marketing materials or other publications of the Organization. I acknowledge and agree publication of said photos confers no rights of ownership or royalties whatsoever. I hereby release the Organization, its contractors, its employees and any third parties involved in the creation or publication of the Organization's publications, from liability for any claims by me or any third party in connection with my or the athlete's participation.
