  • This notice describes how Paperflower Foundation collects, uses, and protects your personal information. Please read it carefully and keep a copy for your records.

  • Who We Are

  • Paperflower Foundation is a nonprofit organization that provides financial support to individuals and families seeking mental health and related services. We work with licensed partner providers to help cover the cost of care for people who need it.

  • Information We Collect

  • Paperflower Foundation collects only the minimum information necessary to determine your eligibility for funding, coordinate payment to your provider, and fulfill our legal obligations. This may include your name, date of birth, contact information, household and financial information, insurance information, and limited billing information such as dates of service and billing codes.

    We do not collect, store, or review your clinical records, therapy notes, session content, or detailed diagnostic information. Your provider's clinical work is entirely separate from our funding function.

  • How We Use Your Information

  • We use the information you provide to determine your eligibility for Foundation funding, communicate with you about your funding status, process payments to your provider through Bill.com, fulfill our reporting and compliance obligations as a nonprofit organization, and improve our programs and services using fully de-identified aggregate data. We do not sell your information.

    We do not use your information for marketing purposes. We do not share your information with advertisers or unaffiliated third parties.

  • Who We Share Your Information With

  • We share your information only as necessary to administer your funding. This includes your provider, to the extent necessary for billing and coordination purposes, Bill.com, our payment processing platform, which maintains its own privacy and security practices, and regulatory or legal authorities if required by law.

    We may share fully de-identified aggregate data — meaning information that cannot be traced back to any individual — for grant reporting, program evaluation, and nonprofit compliance purposes. This may include general information such as age group, diagnosis category, and geographic region. No individually identifiable information is included in these reports.

  • How We Protect Your Information

  • Paperflower Foundation maintains administrative, physical, and technical safeguards to protect your personal information from unauthorized access, use, or disclosure. We store your information securely and limit access to authorized Foundation staff only.

  • Your Rights

  • You have the right to request a copy of the information we hold about you, request a correction of inaccurate information, request that we limit how we use your information, withdraw your consent for us to communicate with your provider at any time, and request that we delete your information subject to our legal retention obligations.

  • How Long We Keep Your Information

  • We retain your information for a minimum of five (5) years from the end of your funding period as required for nonprofit compliance and audit purposes. After that period your records are securely disposed of.

  • Changes to This Notice

  • Paperflower Foundation may update this notice from time to time. The most current version will always be available on our website at paperflowerfoundation.org. If we make material changes we will notify active clients directly.
