Domestic Tour Form

Where a Football Victoria affiliated team is travelling interstate for a football related Tour, a Tour sanctioning request is required to be submitted to Football Victoria to ensure that insurance coverage can remain in place for the duration of the Tour.

A Tour may consist of, but is not limited to, a one-off exhibition match, a series of friendly matches or a tournament.

This form must be submitted to FV at least 3 weeks prior to departure of the Tour.

Sanctioning will not be provided unless all mandatory information included at the end of this application form has been provided.

A letter of approval will be forwarded to the Tour Manager once all requirements have been met.
