NOTICE TO THE MINOR CHILD'S NATURAL GUARDIAN

READ THIS FORM COMPLETELY AND CAREFULLY. YOU ARE AGREEING TO LET YOUR MINOR CHILD ENGAGE IN A POTENTIALLY DANGEROUS ACTIVITY. YOU ARE AGREEING THAT, EVEN IF BIMBI JR SPORTS LLC IS NEGLIGENT, THERE IS A CHANCE YOUR CHILD MAY BE SERIOUSLY INJURED OR KILLED BY PARTICIPATING IN THIS ACTIVITY BECAUSE THERE ARE CERTAIN DANGERS INHERENT IN THE ACTIVITY WHICH CANNOT BE AVOIDED OR ELIMINATED. BY SIGNING THIS FORM YOU ARE GIVING UP YOUR CHILD'S RIGHT AND YOUR RIGHT TO RECOVER FROM BIMBI JR SPORTS LLC IN A LAWSUIT FOR ANY PERSONAL INJURY, INCLUDING DEATH, TO YOUR CHILD OR ANY PROPERTY DAMAGE THAT RESULTS FROM THE RISKS THAT ARE A NATURAL PART OF THE ACTIVITY OR FROM THE ORDINARY NEGLIGENCE OF BIMBI JR SPORTS LLC. YOU HAVE THE RIGHT TO REFUSE TO SIGN THIS FORM, AND BIMBI JR SPORTS LLC HAS THE RIGHT TO REFUSE TO LET YOUR CHILD PARTICIPATE IF YOU DO NOT SIGN THIS FORM.
Acknowledgment and Agreement

Please read and agree to the terms below.
By checking the box below, I confirm that I am the parent or legal guardian of the participant and I have read, understand, and voluntarily agree to the following on behalf of myself and the participant:
- Assumption of Inherent Risks: I understand that soccer is a physical, contact sport. I expressly acknowledge the "inherent risks" including, but not limited to: collisions with players, goalposts, or equipment; slips/falls; and environmental risks such as dehydration, heat exhaustion, and sudden cardiac arrest. I voluntarily assume all risks, including the risk of serious injury, permanent disability, paralysis, or death.
- Physical Condition & Medical Consent: The participant is in appropriate physical condition to participate. I authorize Bimbi Jr Sports LLC to secure emergency medical treatment if I am unavailable, and I accept full financial responsibility for all medical expenses.
- Release and Waiver of Liability: I release, waive, and forever discharge Bimbi Jr Sports LLC, its members (including Stephan Wilde), coaches, and the owners/operators of the facilities or parks where training occurs from any and all claims, demands, or liabilities—including those arising from the negligence of the released parties—to the fullest extent permitted by Florida law.
- Indemnification: I agree to indemnify, defend, and hold harmless Bimbi Jr Sports LLC from any claims or expenses (including attorney fees) brought on behalf of the participant or arising from the participant’s or my own actions.
- Communicable Disease: I acknowledge the risk of exposure to contagious diseases (including COVID-19/Flu) and voluntarily assume all associated risks.
- Rules, Professional Judgment & Administrative Updates: I agree to abide by all program rules, keep all payments current, and respect the professional judgment and decisions of the coaching staff. Bimbi Jr Sports LLC reserves the right to update program schedules, pricing and billing, seasonal structures, and administrative procedures with a minimum of 7 days' notice via WhatsApp/email or the program’s primary messaging group. Continued participation in training following such notice constitutes agreement to the updated terms without the requirement of a new signature.
- Photo and Media Release: I grant Bimbi Jr Sports LLC the irrevocable right to use photographs or videos of the participant for promotional purposes, including social media (Instagram, TikTok, Facebook), website content, and advertisements, without compensation.
I confirm that I am the parent or legal guardian of the participant and have the authority to provide consent on their behalf. I understand that participation in soccer activities is voluntary and involves inherent risks, and that my child participates at our own risk. By entering my initials below and checking the box, I confirm that I have read this waiver, agree to it, and give permission for my child to participate in soccer activities.
