Thank you for your interest in a A Day of Hope 2026! The first step is to complete this registration form.

You will need to provide income and expense information, as well as your children's school and shoe size. Please gather this information before starting the form. If you need to return to this form later, an email with a link to this form has been sent to the email address you provided when you made your appointment.

After you complete this form, you will make an appointment to meet with a representative from A Day of Hope to complete your registration. Details, including the time and location of A Day of Hope 2026 will be given to you at this meeting, which can be in person or online via Zoom.

This form can be completed for up to 7 children. You will need to submit a second form for additional children.
