FAMILY DEDICATION APPLICATION
DEDICATION INSTRUCTIONS AND GUIDELINES:

The Parent (Single/Married) or Legal Guardian must:
- Complete a Dedication application, including signature, in its entirety (below)
- Day of Dedication service: Check your child into KidzTown class at 10:00 a.m. (critical to be on time)
- Attend training class, which will begin promptly at 10:15 a.m. to 10:45 a.m., in the Hall of Miracles (Parent(s)/Legal Guardian(s) must attend training to participate in the Dedication service)
- After training, pick up child(ren) from their class (Pastors will begin dedication service after Praise & Worship)
- You may sit anywhere in the sanctuary that is easy to quickly get to the front
- Family and friends are welcome to take their own photos during the service
- After Dedication, feel free to return child(en) to their class (KidzTown teachers are aware they will be late)
