iPad Usage Agreement & Liability Waiver
As part of the Youth Spring Break Video Production Bootcamp, Hōʻike Kauaʻi Community Media will provide iPads for students to use during class time for video production activities.
By signing below, I understand and agree to the following:
- I acknowledge that the iPad and accessories provided remain the sole property of Hōʻike Kauaʻi Community Media.
- I will ensure that the equipment is handled responsibly and used only for class-related purposes.
- I understand that any damage, loss, or theft of the iPad or its accessories due to negligence or misuse will result in a replacement fee of $1,500, which I agree to pay in full.
- I acknowledge and accept full responsibility for the care of the equipment provided during the course of the program.
By signing below, I also acknowledge that by registering for this class, I am committing my child to attend all scheduled sessions in full.
By signing and initialing this registration form:
- I agree to save and hold harmless Hōʻike: Kauai Community Television, its Board of Directors, Management and Staff from any liability that may arise in the use of Hōʻike's equipment or facilities.
- I understand and agree that the privilege of access to Hōʻike's equipment and facilities is dependent upon adherence to all rules and regulations (expressed and implied) but not limited to those listed below. Failure to comply with the rules and regulations may result in consequences up to and including revocation of all privileges and access as determined by Hōʻike Management.
HŌʻIKE RULES
- Hōʻike equipment and facilities are intended to produce content for cable broadcast on the PEG channels first. No alternative distribution or use is allowed until the program has appeared on Hōʻike. Material generated by Hōʻike equipment but not aired on Hōʻike is restricted from any and all distribution on any platform.
- Project objectives shall remain consistent with a clearly identifiable project "title" for all equipment use.
- Hōʻike, the equipment and facilities are for non-commercial use only- there are no exceptions. If the equipment is found to be used in any commercial or private personal means the Producer agrees to pay full commercial rental charges for the equipment and facilities used in the non-approved production.
- There shall be no calls to action, no mention of charges or pricing or fund raising. Absolutely no commercial content.
- The Producer agrees to bear full responsibility for the safe care and use of the Hōʻike equipment and facilities and agrees to pay for any damages, loss or excessive wear in the event of such occurrence.
Media Release Form
Consent: I voluntarily and without any coercion grant Hōʻike Kaua’i Community Media, Inc. (“Organization”), its representatives, and employees permission to use my image, likeness, and voice, as captured in photographs and/or videos, for promotional, educational, or informational purposes on social media platforms, including but not limited to Facebook, Twitter, Instagram, and YouTube.
Ownership: I acknowledge that all photographs and videos taken by the Organization are and will remain the property of the Organization.
Usage Rights: I grant the Organization the right to edit, crop, modify, or otherwise alter the photographs and videos at their discretion. I understand that the Organization has no obligation to use any photographs or videos taken of me.
Release: I release the Organization, its representatives, and employees from any and all claims, liabilities, or demands arising out of or in connection with the use of my image, likeness, or voice, including any claims for defamation, invasion of privacy, or rights of publicity.
Confidentiality: I understand that the Organization will make reasonable efforts to protect my personal information, but I acknowledge that the images may be shared on public platforms and may be viewed by a wide audience. I agree that the Organization is not responsible for any unauthorized use or sharing of the images by third parties.
Compensation: I understand that I will not receive any financial compensation for the use of my image, likeness, or voice. This release is provided on a voluntary basis and without any expectation of monetary or other compensation.
Revocation of Consent: I understand that I may revoke this consent in writing at any time. However, any photographs or videos taken prior to the revocation may continue to be used by the Organization.
By signing this registration form:
