This is an application for Pleasanton Virtual School.
This does not mean that your student will be accepted into our program simply by filling out this form. Once we have recieved your child's school records and spoken with you over the phone to make sure that you student will be successful in our program then we will send you an email with our official enrollment form that will require you to have your child's social security card and birth certificate.
