    Use this symptom tracker to rate your symptoms and spot patterns over time, alongside what you learn in the book about perimenopause, menopause, and glucose changes.
  • Adapted and inspired by The Greene Climacteric Scale (Greene, J.G., 1998)

  • How to Use This Tracker:

    Once per month, rate each symptom from 0-3.

    Choose:

    0 - Not at all

    1 - A little bit

    2 - Quite a bit

    3 - Extremely

    This form can be opened again and submitted more than once. The Month dropdown is set up to let you choose a different month each time, so you can reassess your symptoms and make separate entries over time.

    Your score helps give context to what you’re experiencing and how it may be affecting your daily life and blood sugar management.

    A total score between:

    0–20
    This is a great time to tune in and stay connected to how you’re feeling. Even mild symptoms matter.

    Use this as an opportunity to notice patterns early and support your body before symptoms progress.


    21–40
    If your symptoms are starting to interrupt daily life or make blood sugar harder to manage, this range is a strong signal to explore support.

    This may be a good time to consider hormonal or non-hormonal options. If you’re already using hormone therapy or supplements, use this as a checkpoint to evaluate what’s helping and what may need adjusting.

    41–60
    This score reflects a higher level of symptom impact and deserves attention. These changes can affect long-term health, metabolic stability, and quality of life.

    This is an important time to speak with a hormone-informed provider and create a plan that supports both your hormones and blood sugar.
