All Candidates MUST

• Be at least 18 years old
• Attend a 1-hour information session
• Must attend all seven training days, on time (lateness is considered an absence)

If you qualify, please complete the registration form below and choose an information session date and time (Wednesday, April 15).  This is mandatory.

Once submitted, you will recieve a confirmation email from jotform which will include the information session location and other details (please check your junk/ spam folder for an email from JotForms).

After attending the information session, you will be notified by phone and email if you are accepted in the class.

Participants who complete the training will receive CEU's (Continuing Education Units), and job placement is based upon class performance and availability.

Requirements for Earning CEUs

• Physical presence during the entire 30 hours course instruction. 
• Completion of training course registration form. 
• Participation in instructor class exercises. 
• Participation and 70% passage rate in instructor class learning assessment.
• Participate in instructor class learning assessments.
  Participation and submission of post course evaluation form (name must me on form for credit)

Course Language
English and Spanish

Prerequisites
Students who register for this course must be able to communicate in the language the course is being offered in. There are no other prerequisites to take this course.
