  • TRAINING GUIDELINES FOR CLIENTS

  • An initial consultation will determine your fitness requirements and help you to set appropriate goals. Your personal aims and lifestyle information will then be used to design an appropriate training program.

    The following when signed will constitute the full agreement between McKenzie Cooper (herein referred to as "trainer') and client.

    The trainer will act as a Personal Trainer in the development of an exercise programme during the agreed sessions.

    In the consideration of the service, the client shall pay in advance according to the session rates charged by the trainer. Refunds are at the trainer's discretion.

  • I   **   ,the undersigned, agree to the following conditions which I will follow to the best of my ability.

  • a. You will be required to have an initial assessment with your Personal Trainer, unless an assessment is deemed unnecessary by the Personal Trainer.

    b. All appointments must be booked with your Personal Trainer

    c. Suitable workout gear and footwear must be worn.

    d. Please inform your Personal Trainer of any medical conditions you have, or any medication you may be taking. If unsure, please ask.

    e. You will be required to give one full calendar month's notice of any cancellation or amendments to your payment plan(s) in relation to Relentless Fitness LLC.

     

  • Declaration of understanding

  • I, the client, agree that I fully understand and agree to abide by the above rules and regulations. I understand that I should inform the trainer of any medical condition and that I may be advised to visit my doctor prior to commencing the sessions.

    I acknowledge that I have either had a physical examination and have been given my doctors permission to participate, or that I have decided to participate in the activity and/or use the equipment or machinery without the approval of my doctor and do hereby assume all responsibility for my participation and activities.

    I release my trainer McKenzie Cooper from any liability for personal injury or other damage I may sustain whilst engaging in any exercise programme suggested by my trainer.

    I agree that any photos or videos taken during my sessions can be used for social media, progress tracking, or any other promotional purposes for Relentless Fitness.

    I acknowledge receipt of this agreement and confirm acceptance of the terms and conditions contained therein.
