  • Please download this spreadsheet and use it to list each individual sample with a unique ID that will allow our lab to identify your samples, as well as the tests you would like run on each sample. There is a seperate tab for Plant Disease samples and Insect Identification Samples. Upload your completed spreadsheet below.

    Note: Spreadsheet is easiest to fill out if you create a copy of it in your own Google Drive and edit it in Sheets. If completing in Excel, you will have to change FALSE to TRUE in order to select a test.

  • Where to Submit Your Samples

    We have made it easier than ever to get your samples to our labs. Whether you are on Oʻahu or a Neighbor Island, follow these steps to ensure your sample arrives safely for testing.

    Drop off your sample at one of the locations below on Monday, from 9:00 a.m. to 4:00 p.m., or Tuesday from 09:00 a.m. to 11 a.m. They will be shipped to our labs on Tuesday afternoons.

     

    For Oʻahu Residents

    Drop off at the CARES Waiawa station (formerly the Urban Garden Center) in Pearl City. (Please call ahead to schedule drop off)

    OR

    Drop off at UH Manoā Shermal Laboratory Rm 115: 1910 East-West Road, Honolulu, HI 96822

    For Neighbor Island Residents

    You can drop off your prepared samples at the CARES Station nearest to you. From there, we will handle the rest. Your samples will be mailed to our UH labs at no additional cost to you.

    • CARES Ho'olehua will not be accepting plant disease samples or insect samples that are NOT preserved in alchool until April 20

    Lānaʻi:Residents on Lānaʻi should contact us directly. We will arrange for an Extension Agent to pick up your sample for shipment via our Maui stations.

    After submitting this form, you will be redirected to our uStore page, but you can just close this tab. If you ordered more than 3 plant disease and/or insect pest identification tests, we will send you a follow up email to pay for the additional samples above the 3 free samples of each type ADSC is providing.

  • After submitting this form, you will be redirected to our uStore page to pay for your sample testing. Please input your name, email, and Job Control number as well as the total price for testing to make payment. You can find your Job Control Number and Total Price for all tests on the confirmation email you will receive after you submit this form.

    Please complete your payment prior to dropping off your samples at one our CARES Offices.
