Please complete the PADI - Medical questionnaire by answering EVERY question on pages 1 and 2. It is an IFS policy that every question on page 1 and 2 is answered. If you answer "yes" to question 3, 5, or 10 on page 1, OR "yes" to any question on page 2, you will need a Doctor's signature to be able to dive at Forfar.

(4) Upload a copy of dive insurance. We recommend purchasing the Regular Individual Membership through Divers Alert Network (DAN). Additional insurance packages through DAN are optional. If you purchase the DAN membership, please submit a screenshot of your membership ID card. **Dive insurance is required for anyone under 18 years old and is recommended for anyone over 18.
