This application is the first step in the enrollment process for Haven 444.

Haven 444 operates as a structured housing and mental health program designed to support women in transitioning from unstable environments into safe, independent living through coordinated care and accountability.

All applicants are required to complete an intake and clinical assessment to determine eligibility, clinical appropriateness, and readiness for a structured living environment. An application is not considered complete until the intake and assessment process has been fully conducted.

Participation in Haven 444 requires engagement in ongoing mental health services as a condition of housing. Services are delivered through structured, treatment-based approaches and include goal-oriented planning to support long-term stability and independence.

Placement is contingent upon clinical appropriateness, program availability, and successful completion of all required enrollment steps.
