I understand that participation in a volleyball tournament involves inherent risks, including but not limited to physical injury, falls, contact with other participants, equipment-related injuries, and other unforeseen risks. I knowingly and voluntarily assume all such risks, both known and unknown, even if arising from the negligence of others.

In consideration of being allowed to participate in this event, I, on behalf of myself, my heirs, assigns, personal representatives, and next of kin, hereby release, waive, discharge, and hold harmless:

• Suncrest Worship
• Lakeside Middle School
• The school district
• Event organizers, volunteers, staff, and sponsors

from any and all liability, claims, demands, losses, or damages arising out of or related to any injury, disability, death, or property damage that may occur as a result of my participation in the tournament, whether caused by negligence or otherwise, to the fullest extent permitted by law.

I certify that I am physically fit and have no medical conditions that would prevent my participation. In the event of an emergency, I authorize event staff to obtain medical treatment for me. I understand that I am responsible for any medical costs incurred.

By signing this document, I agree to uphold the following standards:

• I will demonstrate respect toward all players, referees, volunteers, and spectators.
• I will play fairly and follow all rules of the game.
• I will not engage in unsportsmanlike conduct, including but not limited to verbal abuse, physical aggression, or cheating.
• I will respect the facilities and equipment provided by the school.
• I understand that failure to follow these expectations may result in removal from the tournament without refund.
• I agree to not bring any weapons, drugs or alcohol onto the premises.

I grant permission for the host church and its partners to use photographs or video recordings of me taken during the event for promotional or ministry purposes without compensation.

If the participant is under 18 years of age, a parent or legal guardian must sign below:

I, as the parent/legal guardian of the above-named participant, have read and agree to the terms of this waiver and consent to their participation.

I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, AND SIGN IT FREELY AND VOLUNTARILY.

IF YOU ARE UNDER 18 YEARS OLD YOU MUST HAVE A PARENT SIGN THIS PORTION. BY SIGNING, PARENTS, YOU ARE AGREEING TO ALL ABOVE SECTIONS INCLUDING CODE OF CONDUCT & LIABILITY RELEASE WITH ASSUMPTION OF RISK ON BEHALF OF YOUR CHILD/STUDENT WHO IS PARTICIPATING.
