Member/Client hereby consents to participate in weight training and/or cardiovascular exercise at The Academy, which may or may not be supervised. I understand that physical exercise involves inherent risks, including but not limited to musculoskeletal injury, spinal injuries, abnormal blood pressure responses, and, in rare instances, heart attack or death.
I acknowledge that every reasonable effort will be made to minimize these risks. I confirm that I have read and understand this consent and that my participation is voluntary. I understand that I may discontinue participation at any time. If I have any questions regarding the program, policies, or procedures, I will address them with my trainer immediately.
I agree to the following: (a) I assume all risk of injury and all risk of damage to or loss of property arising from my participation in this program; (b) I release, discharge, and waive any and all responsibility of The Academy from any liability for injury, including death, or damage to or loss of property arising out of or connected with my participation; and (c) I agree to indemnify and hold harmless The Academy, its officers, agents, and employees from any and all liability, claims, demands, actions, losses, and damages arising from my participation in this program.
