• Training with Katelyn LaufenbergLiability Waiver

  • I   understand that participation in any dog training class is not without risk to myself, members of my family, or guest who may attend, or my dog, because some of the dogs to which we will be exposed may be difficult to control and may be the cause of injury even when handled with the greatest amount of care.

    I hereby waive and release Katelyn Laufenberg, Up Front Farms and any adjunct instructors, trainers, volunteers, owners and agents from any and all liability of any nature for injury or damage which I or the dog may suffer, including specifically, but not without limitation any injury or damage which results from the action of any dog.

    I expressly assume the risk of any such damage or injury while attending any training class or other function of Katelyn Laufenberg’s training and any adjunct/guest instructors or volunteers while in the facility, the grounds of the facility, the client’s home, the instructor’s home, or any of the surrounding areas thereto. In consideration of and as inducement to the acceptance of my application for training membership in this dog training class, I agree to indemnifying hold harmless Katelyn Laufenberg, any trainers, volunteer, owners and agents from any and all claims by any member of my family or any other person accompanying me to any training session or function, or while in or on the grounds or the surrounding area thereto as a result of any action by any dog, including my own.

     

  • I acknowledge that I have carefully read this form and fully understand that it is a release of liability. I expressly agree to release and discharge the trainer or instructor from any and all claims or causes of action and I agree to voluntarily give up or waive any right that I may otherwise have to bring a legal action for personal injury or property damage.
