Any volunteer of Fed By Faith located at By Faith Farm under the age of 21 must have a legal guardian to sign this acknowledgment granting their permission to work at By Faith Farm and releasing any liability.
I, the undersigned, hereby acknowledge to Fed By Faith, By Faith Farm and James and Lori Birckhead, that:
I am actively aware of the risk(s) of entering the premises of By Faith Farm located at 7721 Whites Creek Pike, Joelton, TN 37080. By Faith Farm is an actual working farm which risks include, but are not limited to, bodily injury, property injury or death from contact with: farm animals, insects, farming equipment, hand tools, irrigation lines, field stakes, raised beds, electric fences, barbed wire, uneven pathways, slick surfaces, contact with crops or wild plants, which can cause allergic reactions.
I assume all foregoing risk of using property and accept personal responsibility for damages from my use, including, but not limited to, injury, disability, or death.
I release, waive, and discharge Fed By Faith, By Faith Farm and James and Lori Birckhead, the releasees, from any and all liability to me, my heirs, and next of kin for any and all claims, demands, losses, or damages, related to my use of By Faith Farm (James and Lori Birckhead property). I agree to accept full responsibility for any guest whom I bring to By Faith Farm (James and Lori Birckhead property), and will defend and indemnify Fed By Faith and James and Lori Birckhead) against any claim brought by such person.
I further agree to Indemnify and hold harmless, Fed By Faith and Lori and James Birckhead from any loss, liability, damage or costs, including court costs and attorney’s fees, that may incur due to my participation in said activity, whether caused by negligence of releasees or otherwise.
Photo/Audio/Visual Release I hereby grant to Fed By Faith at By Faith Farm all rights and consent to copyright, use, reuse, publish or re-publish, copy, exhibit or distribute all photographs and/or video of myself and my guests to be used for their website and any promotional electronic or printed material without restriction as to frequency or duration of usage and without compensation.
I have read the above waiver and release, understand that I give up substantial rights by signing it, and sign it voluntarily.
