    This form confirms that the caregiver is physically capable of performing the duties required for providing in-home care services including assisting clients with activities of daily living such as mobility assistance, transferes, companionship, meal preparation, and light housekeeping.
  • "I attest that I am physically able to perform my duties required of a caregiver including assisting clients with mobility, standing, walking , lifting, and other activities necessary to safety care for clients in a home environment. I further attest that I do not have any communicable disease that would pose a risk to clients receiving care services. I understand that providing false information may result in termination of employment."

  • Employee Policy Acknowledgement

  • All employees of NurseLink Staffing Group are expected to maintain professionalism, reliability, and high standards of patient care at all times.

  • Hand Hygiene & Infection Control

  • Caregivers must follow proper hand hygiene at all times to prevent infection. Hand hygiene is required:

    • Before and after patient contact
    • After contact with bodiliy fluids
    • Before placing on gloves and after removal of gloves
    • After touching surfaces in the patient environment
    • Before preparing food or assisting with food
  • Acceptable hand hygiene methods include:

    • Washing hands with soap and water for at least 20 seconds
    • Using alcohol-based hand sanitizer when hands are not visibly soiled
  • Attendance Policy

  • Employees must notify NurseLink Staffing Group at least 4 hours prior to the start of their shift if they are unable to work.

    Failure to provide proper notice may result in disciplinary action.

  • No Call / No Show Policy

  • Failure to report to a scheduled shift without notifying the company is considered a no call/no show

    This may result in:

    • Immediate removal from schedule
    • Termination of employment
  • Dress Code

  • All employees must present a clean, professional appearance.

    • Clean scrubs (no wrinkles or stains)
    • Proper hygiene
    • Closed-toe shoes
    • ID badge
  • Confidentiality & HIPAA

  • All patient information must remain strictly confidential.

    Employees must comply with all HIPAA regulations and protect patient privacy at all times.

  • Incident Reporting

  • All incidents must be reported immediately to NurseLink Staffing Group.

    This includes: 

    • Falls
    • Injuries
    • Medication issues
    • Any safety concerns
  • Professional Conduct

  • Employees must:

    • Be respectful to clients and their family members
    • Avoid personal phone use during care
    • Remain focused on patient care at all times
