              • Your dog MUST BE up to date on all of the following vaccines with proof sent before drop off. If your dog doesn't have these, they need to have been given to them at least 10 days before your drop-off date.*
              • Is your dog on Flea and/or Tick preventative? Flea preventative is required. Any dog seen with a heavy flea load upon arrival will be denied service or required to have a flea bath at the owner's expense (+$60 to stay).*

              • What to pack for your dogs stay?

              • * Your dog's food portioned out in plastic bags for each meal unless they’re staying over 4 days. If 5+ days you can bring their food container or bag. 

                * Slow Feeder bowls if your dog uses them.

                * Medications with directions neatly written or typed out (please list below)

                If you opt to bring anything else: beds, blankets, toys, they MUST BE LABELED with your dogs name & your last name. We are not responsible for any lost or missing items.

              • We have daycare here too! Would you like your dog to join group play?
              • We offer both group play and solo turnout. How does your dog get along with other dogs? Please select any that applies. (You can leave more notes at the bottom if needed)

              • Creekside requires a $50-70 deposit to board. After you submit this form, I will be in contact to confirm and collect the deposit. Thanks, Katie Beth
