- 1. The operator is trained in forklift operation and has training documentation on-site.*
- 2. The operator will utilize seatbelt while operating forklift, and will obey speed limits and applicable traffic laws on-site.*

- 3. The operator read the manufacturer’s operation manual, the manual is present and legible in forklift cab.*
- 4. Safety decals and load charts are legible and present.*
- 5. Fluid levels are checked, and equipment inspected for fluid leaks (hydraulic oil, transmission fluid, engine oil, coolant, etc.)*
- 6. Battery inspected for covered terminals, and cable/insulation damage.*
- 7. Overhead guard, mast, lift chains, rollers, and forks inspected for defects (cracked welds, dents, rust, corrosion, missing nuts/bolts, etc.)*
- 8. Tires & wheels are in good condition, with adequate air pressure.*
- 9. Control panel and levers are clean, labels clearly visible (no paint over spray, etc.)*
- 10. Fire extinguisher present, inspected up-to-date, and secured in cab.*

- 11. Braking and steering devices are operating properly.*
- 12. Load-handling attachments are inspected for noise/hesitation during lifting and lowering.*
- 13. Tow hook functioning properly, with smooth engage/disengage, and adequate safety latch.*
- 14. Motion alarms and horns are functional and audible.*
- 15. All Safety Indicator lights are working properly.*
- 16. Seatbelt is functioning properly.*
- 17. All control levers and gauges are in good condition and functioning properly.*

- 18. Work area is clear of drop-offs, holes, bumps, debris, slopes, overhead obstructions, and slippery surfaces*
- 19. Weight of forklift, workers, and other equipment is within floor capacity of work area.*
- 20. Weather and wind in work area have been evaluated and determined acceptable for forklift use.*
