WAIVER AND RELEASE OF LIABILITY CHUNG DO KWAN INVITATIONAL TOURNAMENT I hereby voluntarily submit my application for attendance and participation in the Chung Do Kwan Invitational Tournament,organized by United Tae Kwon Do Society and directed by its affiliated member schools and/or clubs. I, intending to be legally bound, hereby acknowledge that participation in this martial art event is voluntary. I further do hereby waive, release, and hold harmless the United Tae Kwon Do Society, their officers, trustees, and employees, as well as all affiliated organizations, sponsors, organizers, officials, volunteers or employees, for any injury that may be suffered in the course of participation in activities at Chung Do Kwan Invitational Tournament, whether the result of negligence or any other cause. I hereby assumefull responsibility for any and all damages, injuries or losses that I maysustain or incur, if any, while attending or participating. I certify that I have adequate health insurance for my personal needs. In the event of an injury, I understand that medical attention, other than basic first aid, will not be provided. If needed, professional emergency medical response will be solicited on my behalf. Any and all medical expenses are my sole responsibility. I have been advised, and fully understand, that martial arts is a physically challenging activity which by its very nature involves risk. I have been advised of the risks involved in participating in these activities, which include but are not limited to physical stress, contact with other participants, and falling which may result in injuries that include but are not limited to: loss of eye, loss of joint function, sprains, broken bones, concussion, paralysis, and even death. I assume those risks for myself or on behalf of my minor child. I further agree that if my or my child’s participation results in his/her injury or death, that I will not hold any of the above entities liable. I also agree to bring any health or safety concerns to the attention of the instructors. I agree that my performance or attendance at the competition or bothmay be filmed or otherwise recorded or telecast live, and I consent to theuse by the event organizer and/or its affiliatedmember schools and/or clubs of my name; likeness, voice, poses, pictures andbiographical data concerning me, fully or in part and in any form orlanguage with or without other material, throughout the world withoutlimitation, for television, radio, and/or theatrical motion pictures by anydevices now known or thereafter devised, and I waive any compensationtherefore.
