            1. Liability Waiver & Assumption of Risk

            I, the parent/legal guardian of the registered minor, understand that soccer is a physical, contact sport involving inherent risks of injury, including but not limited to sprains, fractures, concussions, and heat-related illness. I voluntarily assume all risks associated with my child’s participation in SISU Soccer Academy LLC programs. I hereby release, waive, and discharge SISU Soccer Academy LLC, its owners, coaches, and staff from any and all liability, claims, or demands resulting from personal injury or illness arising from participation in camp activities.

            2. Medical Consent

            I authorize SISU Soccer Academy LLC staff to seek emergency medical treatment for my child in the event of an injury or medical emergency if I cannot be reached. I agree to be responsible for any costs incurred for such treatment.

            3. Media Release

            I grant SISU Soccer Academy LLC permission to use photographs or video recordings of my child taken during camp for promotional purposes, including social media, the academy website, and marketing materials.

            Code of Conduct

            4. SISU Code of Conduct

            Participation requires adherence to the SISU standards of grit and respect. I agree that my child will:

            • Treat all coaches, teammates, and opponents with respect and dignity.
            • Refrain from bullying, foul language, or disruptive behavior.
            • Wear required safety equipment (shin guards) at all times.
            • Display "Sisu" by giving maximum effort and maintaining a positive attitude.

            I understand that a violation of this Code of Conduct may result in the participant's removal from the program without a refund. By checking the box and signing below, I acknowledge that I have read, understood, and agreed to all terms listed above.
