    INDIVIDUAL REGISTRATION
    All individual person registrations will be sharing rooms with their similar age groups
    • Student (YG, Undergradate, Graduate) - $125
    • Adult - $175

    FAMILY REGISTRATION
    Family registration is designated for immediate families only. A private room will be assigned for each family. Youth Group and Undergraduate aged family members will be assigned a shared room with their similar age groups. (Count family members that are age 4 years and older. Ages 0-3 years are free.)

    • Family of 2 - $ 400.00
    • Family of 3 - $ 450.00
    • Family of 4 - $ 500.00
    • Family of 5 - $ 550.00

  • Medical Release (Cedar Lake Ministries)

    I understand that there are certain dangers inherent in the programs and activities at Cedar Lake Ministries (CLM).Participation in all of these activities is on a voluntary basis only. In consideration of participation in these activities, I do hereby release and forever discharge CLM, its officers and directors, and its employees, agents, and any parties volunteering on behalf of CLM including the activity site owner and operator, from all causes of action, injuries, claims, damages, costs or expenses of any kind, growing out of or related to recreational activities in which I participate; regardless of whether such injury or damage results from the negligence of the Owner (including Owner ’s agents, employees and representatives) or otherwise.

  • Waiver of Liability (CFC)

    In the event that any accident, personal injury or personal property damage should occur, I will hold no liability against Covenant Fellowship Church or any of their staff or representatives from any and all claims, liabilities, suits, action, damages or losses.

  • Parent / Guardian Consent

    I give my child permission to attend the CFC Church Retreat during June 5-7, 2026 at Cedar lake Ministries in Cedar Lake, IN. I am aware that my child as a participant of this retreat, must abide by all rules and regulations and assume responsibility for his/her actions.

  • Medical Consent

    As may be deemed necessary, I give permission for non-prescription medication (e.g. Tylenol, Advil, Mylanta, Etc.) to be issued to my child, and I authorize any medical treatment by a physician or nurse to be administered to my child. In such a case, I will assume the responsibility of all medical bills.
