    New Student Enrollment Form
  • Please note that you will be required to submit the following documents at the end of this form.

    Be sure to have all the required information and paperwork ready before you begin the application process. You may submit them through this form OR bring it with you to our registration office.

    4. Current immunization records 

    Ensure that each document is clear and readable. Submit clear images.

    Missing documents or incorrect documentation will delay the review process.

  • RULES AND REGULATIONS

    1. The minimum age limit for enrollment is 9 years with maximum being 13 for new enrollments.

    2. Submit a birth certificate and photo identification/ID of the person enrolling the student.

    3. Class hours are | Mondays to Thursdays 7:45-3pm | Fridays 8-12pm | Saturdays 9:30-12:30pm |. Class hours are subject to change during different seasons of the year. 

    4. The fees must be paid at the beginning of every term via check Payable to: Al Firdaus Foundation or Zelle/Quickpay: quickpay@alfirdausfnd.org (please note that fees are non-refundable.) 

    5. If a student is traveling or unable to attend for an extended period, the required fees will still apply and must be paid to maintain the student’s enrollment during the absence.

    6. All students are expected to arrive at class before time. Doors may be locked after the above mentioned timings and may not be opened for security and disciplinary purposes. For every 3 days your child is tardy, it will constitute as one unexcused absence. 

    7. 4 unexcused absences per month will lead to a week of in-school suspension. 

    8. In case the child is absent, the school must be informed in advance.
    A student shall be excused from school when the absence is due to his/her illness, for the purpose of medical or dental appointments, and/or to attend the funeral services of a family member of a close relative with a doctor’s note only.

    9. Students must dress appropriately and modestly according to Islamic principles to be able to participate in class. Also, please ensure your child is dressed appropriately and clean for the season and weather conditions. The uniform required is: 

         Boys: thobe/dishdasha with a kufi/topi. 

         Girls: black abaya with a scarf/Hijab that tucks in all the hair 

    10. Students are required to come prepared to school. They should bring their books and supplies to school every day. 

    11. Electronic devices such as phones, tablets, iPads, smartwatches etc. are strictly prohibited. A cell phone is allowed for a valid reason but must be turned in to the office/teacher upon arrival and will be returned during dismissal. If a child is caught with any of the above-mentioned devices during class times, it will be confiscated for one week +.

    12. Parents are not allowed to accompany their children during school hours.

    13. Students will be given daily assignments to complete for homework. Parents are requested to spend a few minutes every day with their child(ren) in completing their homework (if any). 

    14. Foul and bad language will not be tolerated. Bad conduct, behavior i.e hitting, pushing etc. or negatively influencing other studetns may lead to your child being expelled from the school. Bullying will lead to an immediate suspension from school. 

    15. Hair cuts for boys must be in accordance to school rules. Fades/fashion/trending haircuts are not permitted. The hair must always be cut evenly all throughout. It is your responsibility to inform the barber accordingly. Al Firdaus reserves the right to cut/clip hair in school for students failing to comply despite multiple warnings. 

    16. Girls are not permitted to wear makeup, nail polish or artificial nails.

    17. Students are required to bring lunch/snacks from home daily. The school offers a daily meal plan at a significantly discounted cost of $50/month from Fattoush Restaurant.

    18. Your child will be on a probationary/trial period for one month before being fully accepted in to the program.

    19. Warnings are issued initially for not following rules, then suspension for 3 days, then one week, followed by expulsion.

    20. Late pickups are subject to a fee of $10 per late pickup

    As a parent/guardian of this student, I hereby acknowledge that I have read, understood, and accepted the rules and regulations made by Al Firdaus Foundation. I understand that Al Firdaus Foundation possesses full right to make amendments to the rules, regulations, or any other administrative issues set forth; which may or may not be mentioned on the document without prior notice. I give consent to Al Firdaus Foundation to act on our behalf in case of medical or dental treatment if needed. Please inform the office of any health issues beforehand.
