MEDIA CONSENT, RELEASE & WAIVER AGREEMENT
By checking the “I Agree” box and/or participating in any event, program, or activity (the “Program”) operated by NE Showtime (“Organization”), I acknowledge and agree to the following:
1. Consent to Recording and Use
I grant the Organization and its affiliates, partners, sponsors, and designees the irrevocable, perpetual, worldwide, royalty-free right and permission to photograph, record, film, and otherwise capture my name, image, likeness, voice, appearance, and performance (collectively, “Likeness”) in any media format.
2. Use of Media
I understand and agree that the Organization may use, reproduce, edit, modify, distribute, publish, and display such media, in whole or in part, for any lawful purpose, including but not limited to:
Social media content
Marketing and advertising
Promotional materials
Website and digital platforms
Broadcast or streaming platforms
I understand that I will receive no compensation for such use now or in the future.
3. Waiver of Approval Rights
I waive any right to inspect or approve the final media content, including any edited versions, captions, or accompanying text.
4. Release of Liability
To the fullest extent permitted by law, I release, discharge, and hold harmless the Organization, its owners, officers, employees, contractors, affiliates, sponsors, and partners from any and all claims, demands, damages, or causes of action arising out of or related to the use of my Likeness, including but not limited to:
Claims for invasion of privacy
Right of publicity
Defamation (excluding intentional misconduct where prohibited by law)
5. No Obligation to Use
I understand that the Organization is under no obligation to use any media captured.
6. Acknowledgment of Voluntary Participation
I acknowledge that my participation in the Program is voluntary and that I have read and fully understand this agreement.
7. Minors
If the participant is under 18 years of age, a parent or legal guardian must provide consent by agreeing below. The parent/guardian agrees to all terms on behalf of the minor and assumes all related responsibilities.
TEAM CAPTAIN RESPONSIBILITY AGREEMENT
As the Team Captain, I understand that I am responsible for communicating all league information, requirements, and expectations to my team.
I agree to ensure that all players:
Complete their individual registration
Submit required waivers and consent forms
Pay all required fees prior to participation
I understand that players who do not complete registration, payment, and required forms may not be eligible to participate in league games.
I acknowledge that roster management and communication with players is my responsibility, and failure to do so may impact my team’s eligibility or participation.
