Player Participation Waiver and Release of Liability
1. Acknowledgment of Risk
I, the undersigned, acknowledge that participation in basketball activities, including travel to and from events, involves inherent risks, including but not limited to injuries, accidents, and illnesses. I voluntarily assume all risks associated with participation.
2. Release of Liability
I hereby release, waive, and discharge Game Day Sports TN, Todd County Schools, and Clarksville Christian Schools, its coaches, staff, volunteers, and any affiliated entities from any and all liability, claims, demands, actions, or causes of action arising out of or related to any injury, loss, death or damage that may occur as a result of participation in any and all events, including all practices and facilities.
3. Medical Authorization
In the event of a medical emergency, I authorize the coaching staff, event organizers, or designated representatives to obtain medical treatment for the participant. I understand that I am responsible for any medical expenses incurred.
4. Code of Conduct Agreement
I agree that the participant will adhere to all team rules, event policies, and sportsmanship expectations. Failure to do so may result in disciplinary action, including removal from the team or event with no player fee or any other fees refunded by team or organization.
5. Parent/Guardian Signature (if under 18)
By signing below, I acknowledge that I have read and understood this waiver and agree to its terms.
